Pediatric Osteopathic Services What is pediatric osteopathy used for? PLAGIOCEPHALY Pediatric osteopathic therapy for positional plagiocephaly (so-called "flat head syndrome") relies on an exclusively manual, non-invasive, and painless approach designed to release the muscular and fascial tensions that keep the newborn's skull in an asymmetrical position. The effectiveness and success of this treatment are based on the principle that a newborn's cranial bones are still extremely malleable and the sutures have not yet fused. The therapy consists of several sequential stages: 1. Initial assessment and medical history The pediatric osteopath begins by reviewing the child's history, gathering information on the pregnancy, the dynamics of the birth (e.g., multiple births, use of a vacuum extractor, prolonged labor), and daily postural habits. This is followed by a visual and tactile assessment to: Identify areas of flattening and the mobility of individual skull bones. Check for any associated myogenic or positional torticollis, which often compels the newborn to consistently turn their head to the same side. Examine the entire body (spine, pelvis, diaphragm, and abdomen), as postural tensions can manifest all the way up to the head. 2. Manual treatment techniques The actual treatment involves targeted manipulations and pressure, with a force that never exceeds the weight of a fingertip. No abrupt movements or "cracking" techniques are performed. The osteopath primarily uses: Gentle craniosacral techniques: imperceptible pressure applied to the skull to restore proper mobility in the bone sutures and stimulate the head's natural, symmetrical expansion during growth. Releasing tension at the base of the skull and in the neck: to realign the upper cervical vertebrae and relax neck muscles, allowing the baby to turn their head freely in both directions. Balancing internal membranes (meninges): to alleviate intracranial tension that could hinder the harmonious development of the skull. Work on the pelvis and sacrum: to ensure the balance of the entire spine. 3. Education and guidance for parents A crucial part of the therapy takes place at home. The osteopath instructs parents on specific repositioning and handling strategies: Tummy Time: encouraging the baby to lie on their stomach during periods of active wakefulness to strengthen neck muscles and relieve pressure on the back of the head. Active repositioning: alternating the side from which the infant is visually and auditorily stimulated while in the crib or on the changing table, gently encouraging them to turn their head toward the non-flattened side. Handling and carrying: recommending the use of ergonomic slings or carriers and alternating the arm used to hold the baby during breastfeeding or bottle-feeding. Timing and frequency Treatment is most effective when started early—ideally within the first four months of life—when the potential for skull growth and remodeling is at its peak. Generally, an initial course of therapy consists of 3 to 6 sessions, spaced a few weeks apart (approximately 15–20 days) to allow the body's structures time to adapt to the new stimuli. REFLUX Pediatric osteopathic therapy for gastroesophageal reflux (GER) focuses on reducing anatomical tensions that irritate the infant's stomach and esophagus. While it does not cure the chemical reflux itself (which stems from the normal immaturity of the cardiac sphincter), it drastically reduces symptoms such as pain, frequent regurgitation, and associated colic. As with other treatments, this approach is entirely manual, gentle, and painless. The therapy consists of several stages: 1. Initial assessment and medical history The osteopath reviews the infant's history (feeding, type of delivery, sleep patterns) and examines the body to check for: Tension on the vagus nerve: the nerve controlling digestion that passes near the base of the skull. Diaphragm stiffness: the respiratory muscle surrounding the upper part of the stomach. Tension in the abdomen and spine (thoracic and cervical regions). 2. Manual treatment techniques The treatment involves applying very light pressure (as gentle as a caress) to key areas: Release of the base of the skull (craniosacral technique): aims to free the opening through which the vagus nerve exits, thereby improving stomach function and reducing nausea. Diaphragm treatment: gentle techniques to increase the elasticity of the diaphragm muscle, reducing the mechanical pressure that "pushes" acid upwards. Visceral abdominal treatment: very light manipulation of the abdomen to relax the stomach, improve intestinal transit, and reduce gas that increases gastric pressure. Thoracic spine treatment: to rebalance the nervous system responsible for sending digestive signals to the stomach. 3. Practical tips for parents The osteopath suggests daily measures to reduce reflux at home: Feeding posture: keep the baby in the most upright position possible while feeding. Post-feeding care: keep the infant upright for at least 20–30 minutes after a meal (avoiding laying them down immediately). Mattress incline: slightly elevate the head of the crib (by about 15–30 degrees) by placing a wedge or support under the mattress. Timeline and results Benefits regarding sleep and reduced discomfort are often noticeable after just 2 or 3 sessions. Treatment helps the infant experience this growth phase more comfortably while waiting for the stomach to fully mature (typically around 6–10 months of age). COLIC Pediatric osteopathic therapy for neonatal gas colic aims to reduce muscular, fascial, and visceral tension in the abdomen, thereby improving intestinal motility and facilitating the expulsion of gas. While it does not eliminate the physiological immaturity of the digestive system, it alleviates pain and promotes relaxation in the infant. The treatment relies on an exclusively manual approach that is extremely gentle and pain-free. The therapy consists of several stages: 1. Initial assessment and case history The osteopath gathers information regarding feeding (breastfeeding or formula), bowel regularity (constipation or frequent bowel movements), and the times of day when crying is most intense. This is followed by a manual assessment to: Check for tension and stiffness in the abdominal walls and around the intestine. Assess diaphragm mobility; a stiff diaphragm can alter internal abdominal pressure. Examine the lumbar spine and pelvis—areas that share nerve pathways with the abdominal organs. 2. Manual treatment techniques The osteopath employs very light pressure and sustained contact, without exerting invasive force on the infant's abdomen: Visceral abdominal treatment: light stroking and micro-calibrated pressure to relax the intestinal smooth muscle, reduce painful spasms, and facilitate the passage of gas and stool. Diaphragm release: gentle techniques to relieve tension in this respiratory muscle, improving breathing and proper abdominal pressure dynamics. Lumbar and sacral treatment: gentle manipulations to rebalance the autonomic nervous system (sympathetic and parasympathetic divisions), which regulates digestion. Craniosacral techniques: to address the nervous system as a whole, reducing the stress and irritability caused by constant pain. 3. Practical advice and strategies for parents The osteopath demonstrates movements and techniques for parents to replicate at home during crying spells: Specific infant massage: gentle, clockwise circular strokes on the abdomen to help expel gas. Leg flexion: how to correctly perform the "bicycle" movement or gently press the knees toward the chest to aid bowel movements. Skin-to-skin contact: recommending relaxing positions (such as lying face-down along the adult's forearm) that utilize body heat to soothe spasms. Timeline and benefits Initial improvements in sleep quality and a reduction in the frequency of crying spells are usually observed within 2 or 3 sessions. The therapy supports the child through the natural resolution of the condition, which typically occurs around the third or fourth month of life. NECK TENSION AND TORTICOLLIS Pediatric osteopathic therapy for neck tension and torticollis (whether positional or myogenic) aims to restore full cervical mobility to the infant by eliminating the muscle contractures and fascial restrictions that force them to keep their head tilted or rotated toward the same side. The treatment employs an exclusively manual, extremely gentle approach that respects the delicate nature of the infant's cervical structures. The therapy consists of several phases: 1. Initial assessment and case history The osteopath reviews the infant's history (intrauterine position, type of delivery, use of instruments such as a vacuum extractor) and performs a visual and palpatory assessment to: Identify which muscle is tight or shortened (often the sternocleidomastoid). Check the mobility of the upper cervical vertebrae (atlas and axis) and the base of the skull. Rule out associated cranial asymmetries (such as plagiocephaly) or tensions originating in the pelvis and thoracic spine. 2. Manual treatment techniques The osteopath never performs abrupt, sudden maneuvers or forceful traction. Instead, they use light strokes and targeted, very gentle pressure: Myofascial release of neck muscles: gentle, sustained pressure to lengthen and relax tight muscles without causing pain. Craniosacral techniques at the base of the skull: to release tension between the occipital bone and the upper cervical vertebrae, restoring proper head rotation. Balancing tension in the spine and pelvis: to correct compensatory postures the infant adopts throughout their body due to the neck restriction. 3. Practical tips and strategies for parents Home-based care is crucial for consolidating progress and encouraging the baby to use both sides: Sensory stimulation: position toys or lights—or position yourself—on the "difficult" side when speaking to the baby to encourage them to turn their head spontaneously. Management during sleep and diaper changes: alternate the baby's orientation in the crib and on the changing table relative to light sources or the parents' position. Gentle stretching exercises: small, guided passive movements and "Tummy Time" play—demonstrated by the osteopath—to strengthen the neck extensor muscles. Timeline and benefits Torticollis responds very quickly to osteopathic treatment, especially when addressed in the first few months of life. Marked improvements are often seen after just 2 to 4 sessions. Promptly resolving torticollis is the best way to prevent the development of secondary positional plagiocephaly. Osteopathy Services According to the World Osteopathic Health Organization (WOHO) , osteopathy is an established and recognized system of preventive health care that relies on manual contact for diagnosis and treatment. It respects the relationship between the body, mind and spirit in both health and disease: it places emphasis on the structural and functional integrity of the body and the body’s inherent tendency to heal itself. Osteopathic treatment is seen as a facilitating influence to encourage this process of self-regulation. The pain experienced by patients results from a reciprocal relationship between the musculoskeletal and visceral components of disease or strain. As a holistic medicine, from the Greek holos-all, the osteopath considers the individual as a whole. The word “health” itself, from the Sanskrit “sarva”, means wholeness, to come back together in one piece. Again, in Old English, the word “hal”, now commonly known as health-health, means “all”. Osteopathic manipulative treatment (OMT) is the process through which the osteopath approaches somatic dysfunctions. Somatic dysfunctions are classified as anomalies of the musculoskeletal system (ICD-9, code 739) and are identified with the TART parameters: (Tissue texture abnormality, Asymmetry, Restriction of motion, Tenderness). Furthermore, these areas are characterized by a pro-inflammatory state and an alteration of autonomic control. OMT is characterized by different techniques, myofascial release, craniosacral, high velocity low amplitude manipulations (HVLA), BLT techniques (ligament tension balancing), muscle energy techniques (MET), biodynamics, strain-counterstrain. The role of the osteopath is to eliminate obstacles and interferences of the body in order to allow the organism, exploiting its own phenomena of self-regulation and self-healing to regain health. If there is a balance between structure and function, the disease does not arise and this balance will not create compensation zones The organism is constantly looking for comfort, and cannot get used to pain, so it seeks compensation, which is a strategy of the body, through a series of postural changes of ours to not perceive pain.
